申請には、認定番号と受講日が必要になります。
認定番号と受講日は、修了時に発行した認定証に記載されています。
認定証を確認し、正しく記入をお願いします。
When applying, you will need your certification number and the dates you attended.
Your certification number and dates of attendance are listed on the certificate issued upon completion.
Please check your certificate and fill it in correctly.
