・金沢大学総合日本語オリエンテーション＆プレイスメントテストを受験する人は、このフォームから申し込んでください。
If you will be taking a Japanese language course for the first time, you must register for the Orientation & Placement Test from this form.
・*の項目は必ず答えてください。
The asterisk(*) designates required items.
【ATTENTION】
・はじめて日本語を勉強する人も、オリエンテーションをうけるために、ここから申し込みをしなければなりません。
　Students who have never studied Japanese before also must register for the Orientation & Placement Test from this form, otherwise you can not attend the Orientation.
・先学期、総合日本語プログラムの日本語科目を履修した人は、このテストを受けなくてもいいです。
　If students have registered for IJLP course(s) in last semester, you do NOT have to take the placement test again.

＜INSTRUCTIONS 記入上の注意＞
・日本語又は英語で記入してください。
The application form should be filled out either in Japanese or English.
・数字は算用数字を使ってください。Numbers should be in Arabic figures.
1. 名前 NAME (英字 Roman letters)必須

2. 名前 NAME (漢字かな Chinese characters/kana)

(全角文字 full-size of character)
3. 学籍番号 STUDENT ID NUMBER

(半角英数文字 alphanumeric half-size of character)
*もし、まだ学籍番号がわからない場合は、スキップしてもいいです。しかし、授業初日までにわかるようにしておいてください。
If you do not have this yet, you may skip it, but please ensure that you have one by by the classes start.
*教職員、客員研究員の人は、この項目をスキップして、次に進んでください。
Faculty or visiting researchers may skip this form and move directly to the next.
4. eメールアドレス EMAIL ADDRESS必須

「いいえ」と答えた人は、11にいってください。
Students who chose "NO" may skip this form and move directly to 11.

11. もし、オリエンテーション＆プレイスメントテストについて、総合日本語プログラムコーディネーターに特に伝えることがあれば、ここに書いてください。
If you have anything to communicate to the coordinator regarding your participation in the Orientation and Placement Test, please write it down below.
