※休学や留級をしたことがある人は、理由書の提出必須/Those who have repeated a year or taken a leave of absence must submit the statement of reasons.
※兵役休学からの復帰者は、兵役証明書の提出必須/Those who have taken a leave of absence due to the military service must submit the certificate of military service.

■休学・除籍・退学・更新不許可：理由書、あれば診断書
■兵役休学：兵役証明書/服務確認書/除隊(予定)証明書/転役証明書など入隊日と除隊(予定日)が確認できるもの　※英文または自分で日本語訳をつけたもの
■留学：留学先大学の成績証明書など留学をしていたことがわかる書類
