OKOSTA Reservation Request Form

Please check the request which you confirmed.
Requests

The groups more than ten can make a reservation only by phone.
Child who is lower than the 5th grade at elementary school needs attendance of one adult each.
The preschool child participate in cooking.
The application from the travel agency or from the school is possible only by phone.
If there are people who have problems with their legs, or group action, please call us.
Leftovers cannot be packed to take home.
Make sure that you can receive an email from "@otafuku.co.jp".
The cancellation fee is certain percentage of the total price of the reservation as listed below.
seven days prior the date of the reservation: 20%
three days prior the date of the reservation : 50%
on the day : 100%

*The group of members who is lower than the junior high student and not with an adult cannot participate.
*An elementary school of 5th-6th grade student needs participation of more than one adult for three people.
*When there is child who cannot eat one Okonomiyaki, please call.(It is not allowed to pack leftovers to take home.)
