Handling of Personal Information

When participating in this event organized by the The Consortium of Universities in Hyogo, information provided by participants may be used, and photographs and videos may be taken for the purposes of event management, promoting interaction among participants, and public relations activities.

Regarding participant information, a participant list containing names, affiliations, and other relevant details may be created and shared among participants and related parties to the extent necessary for event operations, such as group assignments and facilitating group activities.

In addition, photographs and videos taken during the event may be used on the Consortium of Universities in Hyogo's website, social media accounts, note articles, project reports, and other promotional materials.

Please review the above information carefully. If you agree to the use of your personal information and to the photographing, recording, and use of photographs and videos as described above, please select "I Agree."

【個人情報の取扱いについて】
大学コンソーシアムひょうご神戸が実施する本イベントへの参加にあたり、事業運営、参加者間の交流促進及び広報を目的として、参加者情報の利用並びに写真・動画の撮影を行う場合があります。
参加者情報については、当日の班分けやグループワークの運営等に必要な範囲で、氏名、所属等を記載した参加者名簿を作成し、参加者及び関係者間で共有する場合があります。
また、撮影した写真・動画は、大学コンソーシアムひょうご神戸のホームページ、SNS、note、事業報告書その他の広報媒体に掲載する場合があります。
上記内容をご確認のうえ、参加者情報の利用並びに写真・動画の撮影及び使用について同意いただける場合は「同意する」を選択してください。

【未成年者の保護者同意について】
一般社団法人大学コンソーシアムひょうご神戸が実施する事業（またはイベント）において、未成年者の当該事業（またはイベント）への参加及び個人情報を取得・利用するにあたり、未成年者本人の保護者（法定代理人）からの同意をお願いしています。

1．取得する個人情報
氏名、学校名、学年、住所、電話番号、メールアドレス、緊急連絡先その他事業運営に必要な情報
2．利用目的
取得した個人情報は、「（1）事業運営（またはイベント）及び参加者管理、（2）保護者または本人への連絡、（3）緊急時の対応、（4）アンケート集計及び事業改善」に利用いたします。
3．第三者提供
法令に定める場合を除き、本人及び保護者の同意なく第三者へ提供しません。
ただし、English Village実施に必要な場合は、事前に提供先及び提供内容をお知らせしたうえで提供します。
4．安全管理
取得した個人情報は、漏えい、滅失または毀損の防止その他必要かつ適切な安全管理措置を講じます。
5．開示等
保護者または本人は、法令に基づき、個人情報の開示、訂正、利用停止等を請求することができます。
