2026年度 留学生新年度書類の提出（WEB提出）について（重要）(IMPORTANT) Requirements for 2026 enrollment
※秋入学の方のみ申請ください。This form is only for Fall admissions.
留学生（在留資格「留学」）のみなさんから各種申請を受付ています。Formzu（フォームズ）を利用し外国人留学生データ票申請、授業料減免申請を行ってください。【Formzu】操作マニュアルをよく読んで、以下のとおり提出してください。（学研災付帯学生生活総合保険への加入申請（希望者のみ）は「外国人留学生のための保険についてのご案内」を参照し、Web上で申請してください）
なお、入力中はこまめに入力画面の上部に出てくる「入力内容保存/読込」をクリックするようにしてください。
We collect all documents and applications online, please fill in international student data slip and tuition reduction application using 【Formzu】 (forms).
Read the【Formzu】manual carefully, and fill in required items listed below.
If there is any problem about【Formzu】, please contact the Global Education Center A.S.A.P.
*Note：To take out "Comprehensive Insurance for Students Lives Coupled with PAS" (NOT mandatory), please apply for on WEB with reference to the guidance about "Guidance about the insurance for foreign students".
